We understand that people’s needs change. You may need support with Additional Needs / Disability Adaptations (ANDA) for the first time, or your medical needs may have altered.
If you need changes to your home because of a disability or medical need, please tell us as soon as possible.
Fill in an Additional Needs and Disability Adaptation assessment Form and send it to the National Service Centre, or email it to our secure address: HomeServicesConfidential@pinnacleservicefamilies.co.uk
We also need a report from your Occupational Therapist (OT) or other medical documents explaining what you need. If you don’t have an OT report, or your needs have changed, you must ask your Local Authority for a new one.
